有効な検査証明書の入手が困難な方に対するレターの発行について
令和4年8月19日
新型コロナに罹患し、療養を終えて現在回復しているにもかかわらず、PCR検査を何度行っても陽性判定が続いてしまう方に対し、大使館でレターを発行しています。以下の必要書類(1)～(4)を添付の上、当館領事班（ryoji@sn.mofa.go.jp）までメールをお送りください。
【必要書類】※(1)～(4)全て揃った時点でご申請頂けます。
（1） 旅券（パスポートの顔写真ページ）
（2） ワクチン接種歴がわかるデータや資料
（3） コロナに感染したこと、その後回復したことが確認できる両方の書類
例：病院のリカバリーメモ、又はART検査の陽性結果の写真（撮影日時あり）及び陰性結果の写真（撮影日時あり）、PCR検査結果等
（4） 回復後に受けたPCR検査の陽性結果（厚労省が有効と認める検体及び検査方法に限るhttps://www.mhlw.go.jp/stf/seisakunitsuite/bunya/0000121431_00248.html ）
＜注意＞
※必要書類（3）のART検査の陽性結果等の写真を送付される場合は検査キットに日付を手書きで書き込んだものではなく、携帯電話での撮影日詳細が表示されている画面のスクリーンショットなどをお送りください。画像サイズは圧縮しないでください。
※現在、大使館では非常にたくさんのレター発行に関するお問い合わせを受けており、お電話も平時よりも非常につながりにくくなっています。また、お電話での進捗確認や早期発給依頼を多く頂いていますが人道案件を除き個別のご事情や渡航予定日に合わせた早期発給等に対応することはできません。皆さまそれぞれにご事情があることは十分理解しておりますが公平に対応せざるを得ませんので、何卒ご理解ご協力をお願い致します。
※メール送信の際、添付ファイルの容量にご注意ください。写真など添付ファイルの容量が大きすぎるとメールの送受信にエラーが発生することがあります。メール送信後2業務日以上経っても大使館から返信がない場合は、メールを受信出来ていない可能性がありますので、大使館まで確認の連絡をお願い致します。
※なお、必要書類が整ってからレターの発行までに 3－5開館日かかりますので、陽性反応を受けて帰国便の予約を変更する際は必ず自主検疫期間及びレター発行までの期間を考慮に入れてください。すでに予約しているフライトに間に合わせて発行することはいたしかねます。
The Embassy is issuing letters to travellers who have fully recovered from COVID-19 but still continue to test positive even after taking multiple PCR tests. Please send an email to our Consular Section（ryoji@sn.mofa.go.jp）and attach clear copies of the documents listed below.
【Required Documents】
（1） Passport
（2） Proof of Vaccination Records
（3） Proof of Recovery from COVID-19 Infection
（e.g. Screenshot of TraceTogether/HealthHub records, PCR test/ART positive results (with date taken), Recovery/Discharge memo）
（4） Results of PCR test taken after recovery from COVID-19 (Only limited to samples and test methods approved by the Japan’s Ministry of Health, Labor and Welfarehttps://www.mhlw.go.jp/stf/seisakunitsuite/bunya/0000121431_00248.html)
＜Note＞Please be careful about the size of the attachment file when sending an email. If the size of the attached file is too large, an error may occur in sending and receiving emails. If you do not receive a reply from the Embassy within 2 working days after sending the email, there is a possibility that we have not received the email, so please contact the Embassy for confirmation.
※In addition, it takes up to 5 working days to process your application if the documents are complete. the embassy does not accommodate to early issuance request (with the exception of humanitarian reason.) your understanding is much appreciated.
